I authorize Your Realty Leverage, Inc. to charge my card $1500 upon execution of this agreement and then on the 1st or 15th day of  each month. If the payment is not received within 30 days of the due date, all coaching will be suspended until payment is received.

If payment is not received within 5 days of the due date, all services will be suspended until payment is received and a late fee of 1.5% will be applied. A finance charge of 1.5% per month will be added to all balances unpaid after 30 days. If the method of payment is declined, a convenience fee of $50 will be applied. This authority remains in full force and effect throughout the term of this agreement.

I acknowledge and agree that my contract begins on the date listed above for a period of six months at which time will be moved to a month-to-month basis which may be canceled at any time by either coach or client with 30-days notification in writing. Client may place their account on hold for a maximum of 1 month in a 6-month period. The hold fee is equal to ½ of normal monthly fee and will be automatically charged to the card on file, per the normal payment schedule. Full contract payment will automatically resume the month following the hold, and will be charged to the card on file, per the normal payment schedule. I agree it is my responsibility to notify my coach at least 48 hours in advance if I am unable to meet during the agreed-upon time for my coaching call. I acknowledge and agree that calls may be recorded for training purposes only. Your Realty Leverage, Inc. and Transaction Management Academy makes no warranties, either express or implied, with regards to information provided in coaching or training materials provided. Your Realty Leverage, Inc. and Transaction Management Academy will not accept liability for any loss or damage of any kind that you incur as a result of coaching or training materials provided. All coaching programs and training materials are to support professional development by directing our clients towards productive activities. By providing my phone number on this form, I opt-in to receive calls from Brindley Tucker, and/or any auxiliary companies owned by the above-mentioned parties. Additionally, this coaching relationship is confidential both ways and information shared is proprietary to Your Realty Leverage, Inc. and Transaction Management Academy.
